This is a pre-registration only.  Once you submit this pre-registration form you will receive an email with a short questionnaire within 24 hours that you must complete.  Once we receive your questionnaire, you will receive a follow-up email notifying you of your acceptance and a link for payment.  Payment must  be made within 48 hours to secure your spot.
